Colostomy Reversal Surgery: Restoring Natural Bowel Function

Welcome to our practice! If you’ve been living with a colostomy and are considering a reversal, we’re here to provide expert care and guide you through the process. This is one of my favorite procedures to perform.  A colostomy reversal can greatly improve your quality of life, often through only a few small incisions. Below, you’ll find an overview of colostomy reversal surgery, including both robotic and open approaches.

What is a Colostomy?

A colostomy is a surgical procedure that diverts the colon to an opening (stoma) in the abdominal wall, allowing stool to exit the body into an external bag. It may be temporary or permanent, depending on the underlying condition.

A colostomy reversal reconnects the colon to the rectum or anus, restoring the normal passage of stool. This procedure is typically performed for patients with temporary colostomies once the underlying issue, such as bowel healing after trauma, surgery, or infection, has resolved.

Our Approach to Colostomy Reversal Surgery

Robotic-Assisted Surgery

We specialize in minimally invasive robotic-assisted colostomy reversals, which use advanced technology for precision and enhanced recovery.

  • Procedure: Small incisions are made to insert a high-definition camera and robotic instruments. The surgeon uses these tools to carefully reconnect the colon to the rectum or anus.
  • Benefits:
    • Smaller incisions and less scarring
    • Reduced postoperative pain
    • Faster recovery and shorter hospital stays
    • Lower risk of complications, such as infections
Open Surgery

In certain cases, an open approach may be necessary, especially for complex conditions or dense scar tissue.  However, this is often after an attempt at robot-assisted reversal.

  • Procedure: A larger incision is made in the abdomen to provide direct access to the surgical site. The colon is reconnected to restore bowel continuity.

Who is a Candidate for Colostomy Reversal?

Colostomy reversal is not suitable for everyone. Factors that determine eligibility include:

  • The health of the remaining colon and rectum
  • Overall health and healing from the initial surgery
  • The absence of active infections or significant scar tissue

Your surgeon will perform a thorough evaluation, including imaging and possibly a sigmoidoscopy, to determine whether a reversal is safe and appropriate for you.

What to Expect During the Process

Before Surgery
  • Comprehensive evaluation, including tests and imaging, to confirm readiness for surgery.
  • Preoperative counseling to discuss the procedure, recovery, and potential risks.
  • Bowel preparation may be required before surgery.
During Surgery

The procedure typically takes 2-4 hours, depending on the complexity. It is performed under general anesthesia.

  • In robotic surgery, a console-controlled robotic system provides precise movements.
  • In open surgery, the surgeon accesses the site through a single larger incision.
After Surgery
  • Most patients stay in the hospital for 1-3 days.
  • Gradual reintroduction to a normal diet as bowel function returns.
  • Follow-up care to monitor healing and bowel function.

Recovery and Life After Reversal

Most patients are back to light duty within 1-2 weeks and full activity at 6 weeks. It’s normal to experience changes in bowel habits, such as frequency or urgency, as your body adjusts. You may pass a few bloody bowel movements after surgery that should clear within a few days.

Our team will provide guidance on dietary changes, physical activity, and managing any postoperative symptoms to ensure a smooth recovery.
